Lotta Ytterberg, Division of Physiotherapy, NVS, awarded grant from The Kamprad Family Foundation for Entrepreunership, Research & Charity for the project "Person-centred care transitions for people with complex health conditions: a co-design project".

It was in total SEK 4 000 000.

The project is carried out in collaboration with Danderyds Hospital and Karolinska University Hospital, Home rehabilitation team in Primary Care, Stroke coordinator at Akademiskt Primärvårdscentrum, Strokeförbundet and Afasiförbundet.
