The aim is to develop and integrate these aspects into the stroke prevention program Make My Day.
Seminar description
Make My Day (MMD) is an activity-based, multimodal, interprofessional stroke prevention intervention. Participation in Engaging Everyday Activities (EEA) has the potential to contribute to a sustainable healthy lifestyle and reduce risk factors for stroke. Primary health care has a mandate to work preventively, but there is an urgent need for systematic, person-centered and efficient stroke prevention strategies. The MMD program includes group sessions, individual activity analysis, and activity-based goal setting. The intervention is supported by a mobile phone application that helps participants manage their lifestyle goals.
