Title 

AI in mammography screening - from research to clinical implementation at Capio S:t Göran Hospital and future projects

Abstract 

The Swedish population based breast cancer screening program started in the late 80s and has been a success story with decreasing mortalities, stable cancer detection rates and low recall rates. The program is based on two factors – age and gender. All female genders between 40-74 years old are invited every 18-24 month for breast cancer screening with mammography, four images, two views. 

There are challenges within the screening program – 30% interval cancers and 10-15% large screen detected cancer cases as well as a huge lack of breast radiologists nationwide. It is important to use their time and competence as efficient as possible as well as being as accurate as possible for the proportion of recalled women.

In the first ever prospective clinical study ScreentrustCAD we demonstrated that it is safe to replace one human reader with AI which was implemented clinically in June 2023 at Capio S:t Göran Breast Center with improved screening metrics after implementation. 

Since then we have been working with validation processes of different versions and thresholds of the AI-algorithms, different mammography equipments and different postprocessing techniques.

The next step is to use the information from AI-algorithms even more within the TRAI -trial (AI only for low and high score examinations  and AI plus readers  for intermediate score examinations.

About the speaker 

Dr. Dembrower is a dedicated breast radiologist and her scientific studies have centered around assessing if and how AI can be implemented to improve cancer detection, clinically and in the screening program and breast cancer risk estimation. At Capio S:t Göran Hospital in Stockholm, Sweden she is a Senior Breast Radiologist and Head Physician in the Department of Breast Radiology.  She is also a postdoctoral researcher at Karolinska Institutet, (Department of Medical Epidemiology and Biostatistics) and  Vice President of the Swedish Association of Breast Radiology. She is internationally recognized for her research on the use of artificial intelligence in mammography screening and was a principal investigator in the landmark ScreenTrustCAD trial, which evaluated replacing one of two radiologists in breast screening with AI. Her main research topic now is validating AI-algorithms in different environments as well as pushing the use of AI even further with the three-step TRAI study (retrospective analysis, RCT and clinical implementation and adjustment of thresholds)
