Precision radiooncology exploits the highly targeted nature of radiotherapy to eradicate cancer cells, including cancer stem cells (CSCs). New technological and biological improvements in the identification and detection of radioresistant tumors will continue to drive the evolution of radiation oncology to expand the therapeutic reach and precision of radiotherapy. These include particle therapy, advanced image guidance, and treatment adaptation and in-treatment therapeutic intervention, such as novel biological concepts based on knowledge of the mechanisms of radioresistance. Biomarkers play a key role not only for future strategies for patient selection, but also for precise anatomical targeting, radiation dose prescription, and development of combined modality treatments. Biomarker-driven trials, including trials such as DELPHI and INDIRA currently test such strategies. Such efforts will eventually improve tumor control, survival and quality of life for patients through tailored therapies.
